Your relative (the patient) is either in hospital or a rehabilitation clinic and will be due for release shortly. At first, you should talk to the doctor in charge and the responsible public health and care nurses. They will contact the social services with regard to the transition of care, who will then inform you about specialised nursing care services for outpatient artificial respiration. Of course you may also freely consult the internet, your health care provider, or any other appropriate counselling service.
\nThe next step is to arrange an initial appointment, either through the social services, or directly yourself, with the nursing care service of your choice. From experience, one of more phone calls may be necessary before the first meeting takes place.
\nAn on-site visit will be used in order to personally get to know the patient and the relatives. At this stage, it would be useful to have all details of the patient's medical condition, the respiratory situation, the release date and respective possible date for the transfer of care, to hand. At this point, it is worth mentioning, that it does not matter whether the patient lives in just a one room apartment or in a house.It is the quality of life in the home domestic surroundings that is important.
After an initial verbal agreement with the nursing care service, it will be necessary to make arrangements for the specific care requirements. For this purpose, a care contract will have to be concluded and signed. All details therein will be discussed in advance with the patient and relatives.
\nThereafter, the medical provision of the patient's needs will be organised by the nursing care service. An initial medical prescription will usually be issued by the doctor in charge at the hospital or rehabilitation clinic. On this basis of this prescription, the nursing care service will then contact the health care provider to clarify the costs for a one-to-one care provision (one nurse per patient).<\/p>

Now, it is necessary to organise the required medical aids(respiratory equipment, suction apparatus, healthcare bed, etc.). This is done in consultation with the (rehabilitation) clinic. On request, we will complete all the formalities and organise all the necessary medical care aids. In any event, it is advisable to check the situation at home together with the nursing care service, in regard to the new requirements. Experienced nurses can see at a glance where improvements are necessary and possible.<\/p>

Once the patient is released from hospital, all necessary medical aids, materials and equipment has to be available at home. The patient arrives at home by ambulance in the company of a doctor, and is handed over to the nursing team in the presence of the ward sister. Leitfaden f\u00fcr Angeh\u00f6rige Ihr Angeh\u00f6riger (Patient) befindet sich im Krankenhaus oder in der Rehabilitationsklinik und soll bald nach Hause entlassen werden. Sprechen Sie zun\u00e4chst im Krankenhaus mit dem zust\u00e4ndigen Arzt und der Gesundheits- und Krankenpflegerin. Sie werden den Sozialdienst \/ Pflege\u00fcberleitung \/ Case Management informieren, der Sie dann \u00fcber spezialisierte Pflegedienste in der au\u00dferklinischen Beatmungspflege informiert. Selbstverst\u00e4ndlich k\u00f6nnen Sie sich jederzeit auch im Internet, bei Ihrer Krankenkasse oder entsprechenden Beratungsstellen informieren. Im n\u00e4chsten Schritt vereinbart der Sozialdienst\/Pflege\u00fcberleitung bzw. vereinbaren Sie einen Termin f\u00fcr eine Beratung (\u201eErstgespr\u00e4ch\u201d) mit dem Pflegedienst Ihrer Wahl. Vor dem ersten Treffen sind erfahrungsgem\u00e4\u00df immer ein oder mehrere Telefonate notwendig. Beim Treffen vor Ort geht es zun\u00e4chst darum, den Patienten und Sie pers\u00f6nlich kennenzulernen. Es ist sinnvoll, wenn bis dahin alle wichtigen Informationen \u00fcber den Pflegezustand, die Beatmungssituation, den Entlassungstag bzw. der m\u00f6glichen \u00dcbernahme vorliegen. An dieser Stelle sei erw\u00e4hnt, dass es keine Rolle spielt, ob ein Patient daheim in einem 1-Zimmer Appartement oder in einem Haus lebt. Wichtig ist die Lebensqualit\u00e4t im h\u00e4uslichen Umfeld. Nachdem Sie den Pflegedienst zun\u00e4chst m\u00fcndlich beauftragt haben, wird die Versorgung konkret geplant. Dazu muss ein Pflegevertrag unterschrieben bzw. abgeschlossen werden. Die Details werden vorab gemeinsam mit dem Patienten bzw. den Angeh\u00f6rigen besprochen.Dann muss die \u00e4rztliche Verordnung zur Versorgung des Patienten durch den Pflegedienst organisiert werden. Die erste Verordnung stellt meist der behandelnde Arzt des Krankenhauses bzw. der Rehabilitationsklinik aus. Auf der Basis dieser Verordnung nimmt der Pflegedienst dann zeitnah Kontakt zur Krankenkasse auf, um die Kosten der Eins-zu-Eins-Versorgung (eine Pflegekraft pro Patient) zu kl\u00e4ren. Nun m\u00fcssen die n\u00f6tigen Hilfsmittel (Beatmungsger\u00e4t, Absaugger\u00e4t, Pflegebett etc.) organisiert werden. Dies erfolgt in Absprache mit der (Reha-) Klinik. Auf Wunsch erledigen wir f\u00fcr Sie die gesamten Formalit\u00e4ten und organisieren alle notwendigen (Pflege-) Hilfsmittel. Auf alle F\u00e4lle ist es ratsam, die Situation zu Hause zusammen mit dem Pflegedienst vorab und im Hinblick auf die neuen Anforderungen zu pr\u00fcfen. Erfahrene Pflegekr\u00e4fte erkennen auf den ersten Blick, wo Verbesserungen n\u00f6tig und m\u00f6glich sind. Wird der Patient aus der Klinik entlassen m\u00fcssen zu Hause alle notwendigen Materialien und Utensilien vorhanden sein. Der Patient wird mit dem Krankenwagen und einem Arzt nach Hause gebracht und dort \u2013 im Beisein der Pflegedienstleitung \u2013 von uns aufgenommen\/ \u00fcbernommen.
